Veterinary Referral

Consent from your vet is a legal requirement before your animal can be treated. Please ensure that your vet completes the veterinary referral form below.  Appointments will be cancelled if veterinary referral is not received.

This is required as there many instances in which chiropractic treatment may be inappropriate or needs to be modified.
